DRIVER IDENTITY CONFIRMATION SHEET INSTRUCTIONS

**Follow these instructions to ensure your requests are processed **

WHY DO I NEED TO DO THIS?

The Identity Confirmation process is a simple yet safe process that allows your current or previous employer to make changes to your safety record. In order for your employers to legally act on your Requests for Correction and Rebuttal Notes, they need to know for sure that you are who you say you are. This is for your protection -- so that no one else who has your SSN can request changes to your employment records.

The Identity Confirmation process requires you to provide a copy of your valid Driver’s License and your Social Security Card, along with your signature and date.
They must be copied on to the attached official barcode form. Then you must fax a copy so we can complete the process.

WHAT HAPPENS NEXT?

After you fax the form to us, DriverFacts will review you identification and your signature. If there is a problem with it, we will notify you by email and on your Status Page. The different status' are:

Status Message   Action to be Taken
Confirmation not completed - You must complete the online form.
Waiting for Id Confirmation fax - You must complete & fax the form that printed with this sheet.
Fax Received - DriverFacts is in the process of validating your fax.
ID Confirmation Completed - Your ID Confirmation has been approved.
On hold your Id Confirmation fax was declined - ID was declined, DriverFacts will contact you.

 

Once DriverFacts has completed and approved your ID Confirmation, the employer will be able to process your Request for Correction or Rebuttal.


Note: After you fax your ID Confirmation to us, please allow 2 to 4 hours for us to review it. If you have not received a response from us within 24 hours AND your status has not changed from “ID Confirmation Not Completed”, please re-fax your ID Confirmation to us.


Remember – It is important to check your email or check your Driver Services Status Screen for fastest results.
