Step - Fax Us Your ID Form
Once you have properly filled out the Identity Confirmation Form you see a red box on your User Profile Screen indicating a link you can click to print out your ID Confirmation Fax Form. Click it and print the form

You may Click the "Go back to website" link after you have printed the form.

You will need to photo copy your drivers licence Drivers Licence and Social Security Card placed in the boxes provided on the fax sheet. Then fax this form to us at the fax number provided on the form.
