Step - Enter Identity Information
After you have entered your basic user profile information you will see the Identity Confirmation Form. Here you will fill out information required by the DOT to verify that no one is trying to impersonate you. This allows us to make sure that your information is safe.

Once you have properly filled out the Identity Confirmation Form you will be shown a fax cover sheet which you must print and fax in to us with your Drivers Licence and Social Security Card placed in the boxes provided on the sheet.
